If someone gets your physical ip addy is it possible to actually hack your computer only having this? Thanks for any information guys, really appcreciate it.

Not on most home computers.

This next statement only applies if your computer does not already contain a malicious file listening for inbound connections.

If you fall into both of these categories then yes:
*. You own a computer without service pack 2 installed
2. Your ISP does not block incoming packets on port *** & 445

Otherwise No

Without calling them can I find out if if those ports are in fact blocked? If not, how can I manually block them?

Actually, I see that a third party firewall such as Zonealarm does block incoming packets on these ports. Thanks guys, appreciate the responses.
